General Requirements:

1. Original Passport– passport should have a minimum of two blank visa pages and a minimum 6 months validity and includes all previous passports if applicable.
2. 2 Photos - The frame size must be at least 35 mm x 45 mm (1 3/8″ x 1 3/4″). The photographs must show the full front view of the head, with the face in the middle of the     photograph, and include the top of the shoulders. The size of the head, from chin to crown, must be between 31 mm (1 1/4″) and 36 mm (1 7/16″)
3. Visa application forms duly filled-up IMM5257 and IMM5645 FAMILY     INFORMATION FORM
4. New authenticated NSO birth Certificate , if married, new marriage contract
5. Original Bank certificates w/receipt and Bank Statement
6. Copy of DTI, mayor’s permit, ITR, if self-employed or
7. Original Certificate of Employment, copy of ITR, if employed
8. Service Record
9. Copy of Car Registration
10. Copy of Land titles
11. Proof of pension if retiree
12. Original school certificate for student and ID
13. Affidavit of support for children, notarized
14. Original invitation letter (passport/residence card)
15. Flight Itinerary & hotel voucher
