General Requirements:
- Duly Accomplished Application Form
- 3 pieces Passport Size (4.5 x 3.5 cm) Photograph in White Background.
- Valid Passport (with at least 2 blank pages and must be valid within 6 months from the date of departure)
- Old Passports (if any).
- Original Bank Certificate (Preferably Savings and/or Current Accounts) must be issued not more than 1 month
- Photocopy of the last six (6) months statement of account or passbook of the same bank account reflected in your Bank Certification.
- IF MARRIED: Photocopy of PSA Issued Birth and Marriage Certificate
- IF SINGLE: Photocopy of PSA Issued Birth Certificate
- IF WIDOWED: Photocopy of PSA Issued Birth & Marriage Cert. and Death Certificate of Spouse
- Flight Reservation
- Day-to-Day Itinerary / Hotel Voucher (IF ORGANIZED TOUR)
- IF INVITED: Official Invitation Letter / Proof of relationship /
- Photocopy of Passport of Host/Sponsor (ALL PAGES)
ADDITIONAL: Proof of economic means if financially sponsored by host.
